MH-TCM billing rates

Here is my question:
For those counties billing Blue Cross Blue Shield for MH-TCM what rates are they billing, and what rates is BCBS paying?
Since we started billing them the rate has been $425 for AMH or state rate whichever is higher, and $550 for CMH or state rate whichever is higher.
We are billing our state rate for CMH, but, only getting paid $550.  Just wondering if anyone else is seeing the same thing?

Social Welfare/Rep Payee Services

And, how many clients do you have for this?   This was brought up at the annual conference, and I don’t recall which counties also provided this service.  We are exploring moving away from QuickBooks, as our Social Security Administration audit “suggested”.

Bills for prescriptions/client in secure facility

Can you ask a few counties if they get bills for prescriptions for a child who is placed in a secure detention or correctional facility?  They are not MA eligible if they are in a secure holding?  Do they make the parents pay for the medical bill?

Billing conservatorship after client passes away

We recently had a client who passed away but the conservator is billing for activities performed after date of death.  Do other counties pay for guardianship or conservatorship activities after DOD, is there a cap amount?  In the past we haven’t paid anything after DOD for guardianship, but we have a new director who’s looking for statute that says we can’t.

Paying bills within 35 days

I have a question for those counties that only pay bills once a month at board.  The state auditors have written us up because we are not paying bills within the 35 day under the state statute.  Has anyone else been written up for this?  What have you done to correct this?

Tracking CP Allocation

Becker County is tracking the CP Allocation staff expenses using the net cost (Salary Minus any TCM Revenues earned) method as requested.  The TCM revenues earned are gleaned from SSIS Claim Reports.  Of course, these reports are the full 100% TCM’s, when the reimbursement is actually 50% FFP.  We are thinking we need to adjust the revenues to reflect the 50% reimbursement rate.  We would like to know what other counties are doing to track CP Allocation expenditures.

Cost Effective Health Insurance Billing

Cost Effective Health Insurance (CEHI) Billing-



Our CEHI workers are determining eligibility, the fiscal team approves payment, and we have someone
that bills.  With the new CEHI policies we are running into a problems when we are billing.



Every time there is a circumstance change in the case (some examples include: MA ended/started, new
baby, premium change, number of people covered by insurance changes). Per the instructions, we
need
to make re-determinations of the CEHI eligibility. Typically this change will change the amount approved
for reimbursement and date range in the TPCO screen in MMIS (this is where we get the obligation
number for billing). Our CEHI workers are updating the TPCO screen every time they re-determine
eligibility or end eligibility.  Our clients, once approved for the program, will need to send in paystubs
along with a claim form. The TPCO is ready for billing once the payment is processed. We also make a
detailed case note when the eligibility is determined in MAXIS/METS on approved amount, OB number,
who is covered, insurance amount deducted per check.



Our clients are not very timely about turning in claims forms and paystubs. This is where the issue
comes in. 

  1. The claim comes directly to the Fiscal team to issue payments.  In the meantime the case had to
    be re-determined 1,2 or even 3 times. Per the instructions for the CEHI eligibility worker they
    are to update the TPCO screen payment amount and date but the OB number is staying the
    same.  We are running into problems when billing because the worker is making changes on the
    obligation number before we bill the state for the payments we made. Right now we bill
    monthly but are looking at possibly billing weekly after each payment.  
  2.  In the example below we were unable to bill 2017 because in March 2018 it was determined
    they
    were eligible back to July 2017- 2 different amounts approved. We approved one payment to
    the client from July 2017-Feb 2018 but because we never billed the for 2017. The Obligation ID
    was reused.






MMIS-TPCO will not give you a new obligation number unless a payment has been billed.








This is the last correspondence with the state:



The system isn’t capable of having more than one obligation open on a policy at a time so that is not an
option.  I think your temporary solution of making
the dollar amount high enough works except that it goes against our updated policies and procedures in
regards to tracking purposes.  In other words,  if we were to audit you, what you have listed on the
TPCO screen would possibly not match the dollar amount from the 3767D.  The correct way to proceed
would be to bill the state before you update the obligation #, which requires you to be diligent in your
tracking and record keeping.  And I realize this is why it has been brought to my attention.  I will discuss
this internally next week and let you know what, if anything, we can figure out.  Thank you!

Rep Payee - SSI/RSDI Question

Are you currently Rep. Payee for any children currently in placement that receive SSI or RSDI?

If so, where are you depositing the SSI or RSDI funds coming in from Social Security?  Ex: separate checking account for only these dollars, an account within your general fund, separate fund…

Social Security is indicating to us that these funds need to be deposited into a separate bank account apart from all other funds.  This is new to us as we have always deposited these funds into a chart of account within our Human Services fund. So we are wanting to find out how other counties handle this.

Initial Clothing allowance

  I am wondering how other counties are handling the initial clothing allowances. 



We couldn’t find anything in statute that stated how the process has to work.  We have always reimbursed based upon the receipts provided to our agency, up to the amount that was approved for the clothing allowance.  We currently are working with a facility that would like our office to issue them a check, for the approved amount, and once the facility receives the check the facility will take the child shopping.

Is there a process for determining a time standard percentage on client related time or is it an agency decision?



We are looking for a written procedure or cheat sheet for new staff training and for refresher training for current staff.  Would anyone have one they would be willing to share?



How does your Agency define 100% time reporting?

Daycare initial Payments

Do anyone have a process set up for helping CFC providers with initial daycare costs before they receive their first check?  We have a child placed last week and the provider is in a panic about paying for daycare, to the point that we might need to find another placement.  What do other counties do to offset this expense for the providers, if anything?  Do you cut special checks or tell the cfc provider to figure it out, checks are ran on XX date only.  Help!

MMIS Report help on MN Choices

Good afternoon all,


Does anyone know how to pull a report from MMIS that will show how many MN Choice assessments and reassessments have been completed? Or, how are you tracking this information?  I attached pictures of the MMIS screens where the workers enter a 60 for a DD client and A3 for an LTC client.  We’d like to look back historically to how many we’ve completed and compare that to the amount of time reported in SSIS.

Conservatorships and Guardianships


I’m looking for information on payment of conservatorships and guardianships.
Are there certain situations where the Social Service agency is responsible to pay for the service and others where they are not?
We have a contract for services so when we refer for services I understand we should pay.

Cost of Sexual Assault Exams


8-9-18 Blog post from Blue Earth County….

Per MN Statue 609.35 the county is responsible for the cost of an sexual assault exam. For Blue Earth County, the medical facility will forward the bill to Human Services and it is paid in full. The facility is not allowed to bill insurance. The county may see reimbursement from the insurance company if the victim provides permission. We are considering developing a workflow to support seeking reimbursement from insurance for the cost of the sexual assault exam. Curious if others are doing this and if anybody has any tips to share.

Is there a step by step process that other counties have that involves giving parents opportunities to sign forms that would determine IVE eligibility for their child in placement?  We have many parents who are non-compliant so the IVE determination is either done months after placement, or sometimes not at all. If with court ordering parents to complete the paper work they do not do so. Can a Social Worker sign the forms if we have attempted multiple times and documented that parents have been non-compliant?

Billing for Rape kits

I'm wondering if any other county social service agencies are paying the cost for rape kits and forensic interviews related to criminal cases.  These are sometimes for people who also happen to be social service clients, but often times not.  The statute only says that "the County" is responsible for the cost, but does not identify what department. 



In Red Lake the Sheriff's Office pays for these bills, in Norman County, the Sheriff's Office gets the bills but denies payment and forwards them on to Social Services to pay.  We would like to know how other counties handle this since I'm seeing it both ways in the counties I work for.  Thank you,

Public Health and billing UCare

In Norman County, our public health department is a separate entity, however, they are telling us that they cannot be added as a provider to bill UCare without being listed as a department under the Norman County Social Service office.  We have also had this issue in Availity, where we can see each other's information because we use the same tax ID number as "Norman County" and don't have a separate one for social services.  I'm wondering if this is normal/usual, as in Red Lake County, we do not have our own public health department, so have not run into this before.

72 hour hold billing


We received a bill for a 72 hour hold.  The client had Medicare coverage and the facility received a payment.  The facility now wants us to pay the deductible and co insurance.  One of our supervisors said if they have a contract with Medicare they have to accept that payment in full.  I had asked the woman from the facility if they were suppose to accept that in full like the prepaid health plans and MA and she said no it’s different.  What do other counties do?  Our policy has been we are the payor of last resort.  If the client had Medicare we would not pay anymore and if they were not on MA at the time we would only pay what MA would have paid.

MA Mileage reimbursement question


Question on MA Mileage



We are using the state RUCA formula for submitting MA transportation claims (A0090) and are combining round trips into one service line.  The problem we are running into is that we are being reimbursed only .22 even when the zip code is considered “rural”.



One suggestion we have received is to bill different service lines for each one-way trip instead of combining into a single round trip, using a modifier to indicate the return trip.  There is no modifier listed in the link below.   In the past, trips with the same service date would be rejected.

Scanning Warrant Registers


We are just starting to scan our fiscal department’s paperwork.  We have pretty much everything figured out with one exception… the Warrant Registers because of the signatures.  Everything that we are finding is saying permanent.  Our question to you all is, does permanent mean on paper or can we scan and the scan is treated as the original?  We will be moving into a new space as an agency in December and the less paper we have, the better!  Warrant Registers are the one thing that we have a lot of!

Unloaded Miles Question


Wadena County has been paying unloaded miles to volunteer drivers for those that are on PMAP and MA fee for service.  We have been doing this because we were able to offset our cost by the health plan reimbursement rates.  Now our primary provider (SCHA) does not reimburse the return trip unloaded miles.  We are looking closely at this cost for our 2019 budget.



Does your county pay drivers unloaded miles that are unreimbursed by health plans and if so, how much is your annual cost funded by the county?

Allocating time to grants


Benton County just had a WIC audit and the supervisor salary charged to the grant was denied because I was using an allocation method of her time based on staff time as direct charge instead of indirect.  This is what I received,



In this situation where a supervisor is using the percentage that the subordinates spent on the federal award to claim their own time on the grant does not reasonably reflect that supervisor’s time on the grant, and that allocation method is not reasonable, and therefore the supervisor’s salary/fringe are unallowable as a direct costs. The supervisor’s time can only be charged to the salary/fringe line if it is backed by time studies, dailies, or other documentation that shows the amount of time the supervisor spent working on the WIC grant. If this supporting documentation does not exists, then the supervisor’s time is an indirect cost and should be included on the indirect line”. 



My questions:



How do you charge Public Health supervisor time to the grants, direct or indirect?

If direct, does the supervisor enter time in your system to each grant or are you using some method of allocation?

72 Hour hold billing question


I received a bill for a 72 hour hold.  The person had UCare insurance (not State but private).  The insurance was billed and EOB was attached.  They listed their allowed charge but did not pay and instead put it towards the deductible.  There is also an amount to be written off.  The facility has billed us for the deductible amount.  We were a little confused what they were asking us to pay but she said the amount that was put towards the deductible or what we allow to pay.  I have always been told we do not pay more than the MA rate.  How do I found out what that is?

Reimbursement for meetings

Medicare Premiums question


What are other counties doing that reimburse Medicare monthly premiums to clients?  We currently are verifying that the clients are eligible monthly to make sure we aren’t reimbursing a premium that shouldn’t be reimbursed.

Parental Fees assessed by the state


Morning!  After the AMSSA regional meeting last week and the discussion around parental fees assessed by the state, I want to dig deeper.  I believe that this has been asked before in a difference way, but here it goes…

  1. Parental fees that are not state assessed---what is your method for figuring/determining the fees? 
  2. Has your agency weighed the pros/cons or your method of assessing?
  3. Are you willing to share your policy?

I am asking these questions because we currently use the CS worksheet to determine ours and after the conversation about the state assessed fees, I’m not sure that we are accomplishing our goal by using this method.  We are hitting the families that are normally at rock bottom with huge fees in comparison to their actual means, I struggle with this.  I’d appreciate any feedback, good or bad, that you are willing to share.

Depreciation vs. other for major remodel


We're looking at doing a major remodel.  Can you tell me if it's more cost effective for Social Services to pay increased rent to cover the cost or to expense it out & take the depreciation over a period of time? And why?  Would this apply to the furniture also?

Family Support Grant Question


We are entering our 2018 service arrangements for the Family Support Grant (BC 535).  What kind of proof are other counties requiring when reimbursing the families for funds spent on this?  We are specifically questioning when the family is paying a sitter or for respite and then requesting reimbursement.
