So here is my question, Does anyone have a document created
that they are willing to share, which lists the services counties are mandated
to provide along with the statute citation?

Cook County has a 25 page PDF of Mandated Services that I can e-mail you or I can figure out how to attach a PDF.
Benton has sent a list of services. No statute cited though.
This is the same document that I got when I attended the fiscal part of the New Director training from MACSSA back in 2012. This version I found online today looks more updated than the version I had from my training. It also looks a lot like Julie’s attachment but also maybe more updated?
